Complexities of the Healthcare System, first sample

The experiences of all sorts and colors from my work as a  Medical Interpreter can be as numerous as stars, specially when a few interpreters sit down and start learning from each other...How do you say this...? What country is this phrase from...?

However, one of the things people is somewhat limited to is the fact the medical community, by that I mean providers, have a separate language, it is called Medicalese. Attorneys have their own too, and pretty much every profession addresses concepts, theories and protocols in a very technical way. It is natural at the professional level, and in a world where sub-specialization is more and more the daily bread, that technical language only increases its presence and complexity.

Las experiencias de tdos tipos y colores de mi trabajo como Intérprete Médico pueden ser tan numerosas como las estrellas del firmamento, especialmente cuando unos cuantos interpreters nos juntamos y empezamos a aprender el uno del otro...¿Cómo se dice...?, ¿De qué país es esta frase...?

Sin embargo, una de las cosas que en parte limita a las personas, es el hecho que la comunidad médica (me refiero a los profesionales de la salud) tienen su propio idioma, éste se llama Medicalés. Los abogados tsambién tienen el suyo propio, y casi todas las profesiones tratan los conceptos, teorías y protocolos en una manera muy técnica. Es algo natural a nivel profesional, y en un mundo donde las sub-especializaciones están a la órden del día, el idioma técnico va a incrementar su presencia y complejidad.

Having said that, the best explanations are the ones that simplify complex and complicated topics to the essentials to facilitate understanding. In that sense, I think that the healthcare system should not escape from that, on the contrary, it could solve many problems and reduce unnecessary financial and emotional expenses.

So, I felt like sharing some of the things that I have observed that could easily be fixed if we remembered that not every person accessing the healthcare system speaks Medicalese.

Habiendo dicho eso, las mejores explicaciones son las que simplifican los temas complejos y complicados a lo esencial para facilitar su entendimiento. En ese sentido, creo que el sistema de cuidados de la salud no debería escapar a eso, al contrario, podría ayudar a resolver muchos problemas y reducir gastos financieros y emocionales innecsarios.

The OTC Experience

A patient goes to the emergency department of a local hospital and arrives with multiple symptoms (runny nose, body aches, headache, fatigue, pressure in the ears, etc.) that are collected and analyzed by the medical personnel. After several hours waiting in the hospital, the physician comes to the patient and tells her:

Un paciente va a la sala de emergencias de un hospital local y se presenta con múltiples síntomas (nariz que le chorrea, malestares, dolor de cabeza, fatiga, presión en los oídos, etc.) que son recopilados y analizados por el personal médico. Luego de varias horas en el hospital, el médico se dirige al paciente y le comenta:

Medical Provider:

"Ma'am, we got your labs back and it seems that you are having a viral infection. As of now, there's really nothing we can do other than let it run its course. You can take acetaminophen every 4-6 hours as needed.

Do you have any questions? Otherwise, the nurse will bring your discharge papers."

Profesional de la Salud:

"Señora, ya tenemos los resultados de sus análisis de laboratorio y parece ser que usted tiene una infección viral. Ahora mismo no hay nada que podamos hacer más que dejar que siga su curso. Usted puede tomar acetaminofeno cada 4-6 horas según lo necesite. ¿Tiene alguna pregunta? De lo contrario, la enfermera le traerá los documentos para darle de alta."

Patient:

"So, I've been waiting in the ER for over 5 hours and all I get is pain medicine? No antibiotics?. I'm not coming back here..."

Paciente:

"O sea, he estado esperando en la sala de emergencias por más de 5 horas y todo lo me da es un remedio para el dolor?, ¿Nada de antibióticos?. Yo no vuelvo a regresar a este lugar..."

This is a typical scenario that as a Medical Interpreter I have witnessed, maybe it's the biologist in me that wants to speak up and say that Viruses are not living things therefore cannot be killed, or destroyed with antibiotics. Antibiotics are for bacteria, not viruses. The other thing is that viral infections indeed have to run their course, and you can only attempt to mitigate the symptoms.

Este es un escenario típico, como Intérprete Médico, de lo que he sido testigo, y quizás sea el biólogo dentro de mí que quiere hablar y decir que los Virus no son seres vivos, por lo tanto no se les puede matar, o destruir con antibióticos. Los antibióticos son para las bacterias, no para los virus. Lo otro que es cierto, es que las infecciones virales tienen que seguir su curso, y lo único que se puede intentar es mitigar los síntomas.

Medicalese then, is not complex only because of its linguistic difficulty and high level jargon and acronyms, but also because it involves specific knowledge that is far from being common knowledge.

Unfortunately, the consequences of this common ER encounter, are not in anybody's best interests. The patient may think the doctor doesn't know how to treat the viral infection, or doesn't care much, I don't know which is worst. Based on this experience, that patient may diminish the trust deposited in medical providers' services from there on, and wait until the last minute to seek medical attention again. That is certainly not good, given the fact that the older we get, the more maintenance our body machine needs, and if one or more of the components are not properly taken care of, a system shutdown is likely to happen.

Entonces, Medicalés no solo es complejo por la dificultad lingüística y el alto nivel de jergas y acrónimos, sino también porque eso también involucra tener conocimientos específicos que están lejos de pertener al conocimiento común.

Desafortunadamente, las consecuencias de este encuentro común en la sala de emergencias, no le conviene a nadie. El paciente podría pensar que el doctor no sabe cómo dar tratamiento a una infección viral, o que no le importa, de hecho no sé cuál de esas dos razones es la peor. En base a esta experiencia, el paciente puede reducir la confianza depositada en los profesionales de la salud de ahí en adelante y esperar hasta el último minuto para buscar atención médica nuevamente. De hecho eso no es nada bueno, considerando que a medida que envejecemos, nuestra cuerpo como cualquier máquina tiene más necesidades y mantenimiento, y si uno o más de esos componentes  no se mantienen bien, es probable que nuestro cuerpo podría dejar de funcionar.

The Need for a Universal Translator

Nothing is more human than spoken language and diversity. A clear proof of that, is that over thousands of years we have developed not one, but perhaps thousands of spoken languages. Many of them are extinct by now, some others are endangered, but all are constantly changing, and over time, evolving. Check: Ethnologue.

Nada es más humano que el lenguaje hablado y la diversidad. Una prueba clara de ello, es que a lo largo de miles de años hemos desarrollado no uno, sino quizá miles de lenguajes hablados. Muchos de ellos ya están extintos, algunos otros están en peligro de extinción, pero todos están en constante cambio, y con el tiempo, evolucionan. Revisar: Ethnologue.

One of my friends is a linguist, and he taught me to understand that languages are living entities, more powerful than memes and ideas. Now that I work immersed completely in the diversity of languages and the richness of regionalisms and the intricacies of the day-to-day dialogue, I cannot resist but being puzzled by the fact that some people can think that we can cover the nuances of spoken language with a single universal translator. I think no one can deny the utter need for connecting people through a common language. But that has not been achieved by machines, electronics or digital technology. Not even now, and perhaps never. Because when we find a universal language translator, we'll probable be in the capacity to produce a "human translator", to understand humans...input context, language, culture, age, education, experiences, mood, health and time of the day (to cite some circumstances), and the "human translator" will give you an explanation of why the person is acting as it is. Unlikely!

Tengo un amigo que es lingüista, él me enseñó a entender que los lenguajes o lenguas son entidades vivas, más poderosas que los memes y las ideas. Ahora que yo trabajo inmerso completamente en la diversidad de las lenguas y la riqueza de los regionalismos y las complejidades del diálogo del día a día, no puedo resistir sentirme intrigado por el hecho de que algunas personas pueden pensar que se pueden cubrir los matices del lenguaje hablado con un simple traductor universal. Creo que nadie puede negar la imperativa necesidad  de conectar a las personas a través de un lenguaje en común. Pero eso no se ha podido lograr a través de las máquinas, la electrónica o la tecnología digital. No ahora, y quizá nunca.
Porque cuando encontremos un traductor universal del lenguaje, probablemente nos encontremos en la capacidad de producir un "traductor humano", para entender a los humanos...ingrese el contexto, lenguage, cultura, edad, educación, experiencias, estado de ánimo, salud y hora del día (por citar algunas circunstancias), y el "traductior humano" nos dará  una explicación del porqué la persona está actuando como tal. Improbable!

There are many attempts to standardize the translation of written language, and even though technology has driven us to a great stage of development, it'll be difficult that a silicon wafer can translate common sense, double-sense and the message behind the phrases that don't make sense. Let's leave that to our internal techmnology, the brain.

Exsiten muchos intentos para estandarizar la traducción del lenguaje escrito, e inclusive con la tecnología poniéndonos en una gran etapa de desarrollo, será muy difícil que un wafer de Silicio pueda traducir el sentido común, el doble sentido, y el mensaje detrás de las frases que no tienen sentido. Dejemos eso a nuestra tecnologíaa interna, el cerebro.

Abundance and Future

This blog has been dormant for some time. The people and pieces necessary for an on going posting routine are now in place. In order to celebrate our return to the blogosphere it's opportune to discuss about good and new possibilities. Even though we hear constantly about this from small areas in society (technology, culture, medicine, etc), it is seldom done on a global basis, the so-called "big picture".

Este blog ha permanecido durmiente por algún tiempo. Pero las personas y las piezas necesarias para una rutina de publicación continua están ahora en su lugar. Para poder celebrar nuestro retorno a la blogosfera creemeos oportuno discutir sobre nuevas y buenas posibilidades. Incluso cuando escuchemos sobre esto constantemente desde pequeñas áreas en la sociedad (tecnología, cultura, medicina, etc), en raras oportunidades se lleva a cabo de forma global, o con la llamada "visión general".

So our best way to do it is by celebrating the infinite possibilities that can arise from human imagination, work and global effort. We'll start by commenting on a TED conference video, Peter Diamandis: Abundance is our future.

Así que la mejor forma de hacerlo es celebrando las posibilidades infinitas que pueden surgir de la imaginación humana, trabajo y esfuerzo global. Empezaremos comentando el video de la conferencia de TED, Peter Diamandis: Abundancia es nuestro futuro (subítulos disponibles en muchos idiomas).

This conference gives us a reminder of the world we live in now compared to the world our ancestors lived in 100 years ago. But the most important of all, what will happen in the very near future that we can't ignore. More than 3 billion new people will have internet access within the next 8 years.  Most of them (if not all) will come into the world wide web from developing and underdeveloped countries, countries with a gigantic cultural and linguistic diversity, richness and potential. Not the best nomenclature for nations with reduced number of opportunities, and although having access to the internet doesn't secure higher standards of living by itself, it'll definitely empower people with the capacity to control and further their knowledge, learning and  interests. That's good news for the economy, but more importantly to our world in general.
Esta conferencia nos recuerda el mundo en que vivimos en comparación con el que vivían nuestros ancestros hace 100 años. pero lo más importante de todo, es lo que ocurrirá en el futuro inmediato y que no podemos ignorar. Más de 3 billones de personas nuevas tendrán acceso a la internet dentro de los siguientes 8 años. Muchas de estas personas (si no todas) ingresarán a la web mundial desde países en vías de desarrollo o subdesarrollados; países con una gigantesca diversidad, riqueza y potencial linguístico y cultural. No es la mejor nomenclatura para las naciones un número reducido de oportunidades, y a pesar de que el acceso en sí a la internet no asegura estándares de vida más altos, si podrán (definitivamente) darle el poder y la capacidad a la gente de controlar y mejorar su conocimiento, aprendizaje e intereses. Esas son buenas noticias para la economía, pero más importante, para nuestro mundo en general.

As a Medical Interpreter exposed to cultural and linguistic diversity on a daily basis, it is beyond my fascination to be part of the acculturation process of countless immigrants in the United States, helping them survive in this complex society one day at a time. And who knows, maybe I was lucky enough to know the future best protein folder, nanotechnology engineer, musician or politician that can help us solve our future problems.

Como un Interprete Médico que está expuesto a la diversidad linguística y cultural a diario, esto sobrepasa mi fascinación de ser parte del proceso de aculturación de incontables inmigrantes en los Estados Unidos, ayudándoles a sobrevivir en esta sociedad compleja un día a la vez. Quién sabe, de repente tengo la suertede conocer al mejor plegador de proteínas, ingeniero nanotecnólogo, músico o político que pueda ayudarnos a resolver los problemas del futuro.

English Proficiency - July 31st

English proficiency is the central idea of becoming part of the Canadian, US or UK culture, depending on where you are. People should be able to acquire it in a prudent amount of time. It is in the meantime that an interpreter is needed to secure that the communication and information is relayed accurately between the communicating parts.

El conocimiento del idioma Inglés es la idea central de asimilarse a la cultura canadiense, americana o británica, dependiendo de dónde esté. Las personas deben adquirir dicho conocimiento en un tiempo prudencial. Es en el entretiempo que el interprete se hace necesario para asegurar que la comunicación e información sean pasados de forma correcta entre las partes comunicantes.

Bridging the Gap

There are very few frustrating situations in life where one can do little or nothing for it. Communicating effectively and accurately in a medical setting in a language different than your native one, is at the top of the list.

As has been commented in previous posts, Cultural Competency and Language mastering, are the main components of a good Medical Interpreter. But how do you get those assets?

There are very few certifications in the US that are nationwide standards in the field of interpreting. One of them is Bridging the Gap, a certification course that offers 40 hours of intensive training sessions that will let you dive into the cultural diversity, modes of interpretation, ethics and medical terminology, all of this necessary to start practicing in the field of Medical Interpretation. It is the first step in a long and continuous list of training sessions, new techniques and certifications, that any knowledgeable Medical Interpreter should go through to be able to face the daily challenges of the healthcare system.

Hay muy pocas situaciones frustrantes en la vida en donde uno puede hacer poco o nada. Comunicarse de forma efectiva y precisa en un entorno médico en un idioma distinto al materno, se encuentra en el top de la lista.
Como ha sido comentado en entradas previas, la Competencia Cultiural y el Dominio del idioma, son los componentes principales de un buen Interprete médico. Pero, cómo se consiguen éstas destrezas?
Son muy pocas las certificaciones en los EEUU que son considerados estándares nacionales en el campo de la interpretación. Uno de ellos es Bridging the Gap, un curso de certificación que ofrece 40 horas de intenso entrenamiento, que le permitirán zambullirse en el mundo de la Diversidad Cultural, modos de interpretación, ética y terminología médica, todo esto necesario para empezar a laborar en el área de la Interpretación Médica. Es el primer paso dentro de una larga lista de sesiones de entrenamiento largas y continuas, nuevas técnicas y certificaciones con las que cualquier Interprete Médico debería contar para poder enfrentarse a los retos diarios del sistema del cuidado de la salud.

Language standards in hospitals

Mentioned in a previous post (Organizations), I wrote about JCAHO as the institution in charge of accrediting and certifying hospitals in the US. JCAHO published a very important document in april this year listing the reasons, resources and standards for US hospitals. I will cite the initial paragraph of the document because it is of great importance:

"The Joint Commission views the issue of the provision of culturally and linguistically
appropriate health care services as an important quality and safety issue and a key element in
individual-centered care. It is well recognized that the individual’s involvement in care decisions
is not only an identified right, but is a necessary source of accurate assessment and treatment
information. The Joint Commission has several standards that support the provision of care,
treatment, and services in a manner that is conducive to the cultural, language, literacy, and
learning needs of individuals".

Mencionado previamente en una entrada anterior (Organizations), escribí sobre la función de JCAHO como la institución a cargo de la acreditación y certificación de hospitales en EEUU. JCAHO ha publicado en abril de este año un documento que lista las razones, recursos y estándares para los hospitales de EEUU. Voy a citar el párrafo inicial del documento porque es de gran importancia:

" La comisión revisora visualiza el tema de la provisión de servicios cultural y linguísticamente apropiados del cuidado de la salud, como un tema de segurudad y calidad importante, y un elemento básico en el cuidao centrado en el individuo. Es muy reconocido que involurar al individuo, o paciente, en las decisiones de su propio cuidado no sólo es un derecho identificado, sino también algo que es necesario para una evaluación precisa y tratamiento informado. La comisión revisora tiene diversos estándares que sostienen la provisión de cuidados, tratamiento, y servicios en una forma que contribuye con las necesidades de aprendizaje, culturales, de idioma, y alfabetismode los individuos".

Languages

Many are the languages in the world. Many are the languages that are extinct, and many more sublanguages are the ones in the creation yet. Nothing is more impressive than to observe the evolution of something nonliving in real time, as the evolution of languages.

We help in that evolution everyday, in each of our countries and in every new word we speak, modify and improvise.

It is extremely difficult to keep up with all the languages of the world, when needing help just visit Ethnologue, a great website that will help you find the different languages spoken in every country. Now, if you are in very good mood, try a Spanish slang website. Believe it or not, speaking Spanish is great, but you must be very careful when speaking it in a country that is not yours, because even though you might be using a common word, you might be implying something very funny, uncomfortable or dangerous. Not to be too worried about, but just keep it in mind, language sensitivity and cultural awareness.

Muchos son los idiomas en el mundo. Muchos son los idiomas que se han extinguido, y muchos más los subidiomas que se encuentran en creación ahora mismo. Nada es más impresionante que observar la evolución de algo no viviente en tiempo real, como la evolución de los idiomas.

Nosotros contribuimos en esa evolución todos los días, en cada uno de nuestros paises y en cada una de las palabras que hablamos, modificamos e improvisamos.

Es extremadamente dificil estra al día con todos los idiomas del mundo, cuando necesiten ayuda simplemente visiten Ethnologue, una página web que le ayudará a encontrar las diferentes lenguas de cada país. Ahora, si está de buen humor, pruebe con una página de jerga en habla hispana. Creanlo o no, hablar Español es grandioso, pero hay que tener cuidado cuando se habla en un país que no es el suyo, ya que podría estar utilizando una palabra común, pero podría estar implicando algo muy gracioso, incómodo o peligroso. No hay preocuparse, tan sólo tenerlopresente, sensibilidad del idioma y conocimiento de la cultura.

Organizations

There are some organizations that are in charge of protecting those who cannot communicate efficiently, also called LEP (Limited English Proficient). Among the most important organizations with such role are the official ones, being JCAHO at the top, because is the organization in charge of accrediting and certifying over 15,000 hospitals in the United States of America, having communication and equal linguistic access as one of its top priorities. On the private side, the Robert Wood Johnson Foundation is one of the leaders in promoting equality in the access to healthcare, not only in terms of language, but in many more fronts too, verified by the creation of programs like Speaking Together.

Existen algunas organizaciones que están a cargo de proteger a aquellos que no pueden comunicarse efectivamente, también denominados LEP (inexpertos en el inglés). Dentro de las organizaciones más importantes con un rol oficial, tal como JCAHO estando al tope, ya que es la organización que se encarga de acreditar y certificar más de 15,000 hospitales en los Estados Unidos de América, tiene a la comunicación y acceso linguístico igualitario como una de sus más altas prioridades. En el sector privado, la Fundación Robert Wood Johnson es una de las líderes al promover igualdad en acceso al sistema de salud, no sólo en términos linguísticos, pero en muchos otros frentes también, verificados por la creación de programas tales como Hablemos Juntos.

This video will give you a general idea of why Medical Interpreting is a necessity, not only for those who cannot communicate effectively in English, but also for economic reasons because medical mistakes cost hundreds of millions of dollars. And an office visit, medical procedure or surgeries, are perfect situations to make mistakes if the medical personnel doesn't have a proficient language bridge to overcome the language barrier.
Este video le dará una idea general de porqué la Interpretación Médica es una necesidad, no sólo para quienes no pueden comunicarse efectivamente en inglés, pero también por razones económicas, ya que los errores médicos cuestan cientos de millones de dólares. Una visita al médico en su consultorio, un procedimiento médico o cirujías, son las situaciones perfectas para cometer errores si es que el personal médico no tiene un puente competente de idioma que le permita sobreponerse a las barreras linguísticas.

Executive Order 13166

Government assistance has been in place, as a presidential order since august, 2000. This executive order assures equal access to US government's services. Which means that any organization directly, or indirectly related to federal funding, must provide equal access to such services. It entails the implementation of handicap access, and language assistance, to name a few. This is verified through the available multilanguage federal documents, and the use of interpreters, either by phone or in-site.

Probably the best organized interpreters association, is the one for the deaf community (hearing impaired). Incredible organization that cares for members and overall community.

Those are excellent examples of organization.

La asistencia del gobierno ha estado indicada, como una orden presidencial, desde agosto del 2000. Esta orden ejecutiva procura el acceso igualitario a los servicios que que el gobierno de los Estados Unidos proporciona. Esto significa que cualquier organización directa, o indirectamente relacionada a los fondos, o subvenciones federales, debe proveer un acceso igualitario a dichos servicios. Esto conlleva la implementación de acceso para discapacitados, y asistencia linguística, por nombrar algunos. Este se puede verificar a través de la disponibilidad de documentos federales en diferentes idiomas, y por el uso de interpretes, ya sea vía telefónica o en persona.

Probablemente la mejor y más organizada asociación de interpretes, es la de la comunidad de sordos (discapacidad auditiva). Una organización increible que cuida a sus miembros y a la comunidad en general.

Esos son ejemplos excelentes de organización.
